CLINICAL INFORMATION

Chyle is a milky white fluid obtained in the bowels (small intestine) during primary metabolism during digestion which comprises tiny fat droplets, which carry fats and proteins. These lymph vessels carry chyle to the bloodstream and these fat-rich droplets are transported where they are needed. Chyluria (also called as chylousuria, galacturia) is a rare condition in which lymphatic fluid drains into the kidneys and turns the urine milky white. Chyluria is a disorder that occurs due to obstruction in the lymphatic flow, hence resulting in the abnormal circulation of lymph fluid that may drain into the kidneys, ureter, urinary bladder etc. It can be caused due to either infectious diseases and also due to non-infectious causes. Some of the pathological conditions that can lead to chyluria include lymphatic flow disorder (non-infectious cause) such as lymphangiomatosis (congenital lymphatic malformations), congenital syndromes like Gorham-Stout, Noonan, Turner’s syndrome, lymphatic to vesical fistulae, kidney transplant, pregnancy, childbirth, partial nephrectomy or thermal ablation of kidney lesion, obstruction or stenosis of thoracic duct, nephrotic syndrome, other causes like diabetes mellitus, abscess etc and infectious causes encompass ankylostomiasis, trichiniasis, taenia nana, Wuchereriabancrofti that cause lymphatic filariasis – elephatiasis. Chyluria is often caused by filariasis due to the parasite Wuchereriabancrofti (nematode) which circulates in lymphatic channels. These thread-like nematode parasites accumulate in the peripheral lymphatic vessels and block the flow, thus increasing the pressure within the lymphatic vessels. Thus the increase in the lymphatic pressure forms a fistula and/or collateral flow of the lymph from the intestinal lymphatic vessel to the kidneys and ureter. Moreover, the developed urinary fistulae get ruptured and therefore the contents of the lymphatic vessels find access to urine and hence WBCs, fats, and fat-soluble vitamins are expelled into the urine. Clinical manifestations of chyluria include tiredness, edema, increase in body weight, pain during urination, frequent urination, blood in the urine (hematuria), pain in the lower back on one side etc. The untreated condition can lead to complications such as fat-soluble vitamin deficiency and malnutrition. Chyluria Test is performed on urine sample specimen that measures fat content (chylomicrons), triglyceride, cholesterol, microscopic analysis for parasitic nematode (Wuchereriabancrofti) etc, in the urine. Additional tests include imaging studies such as X-rays, CT scans, MRI, Ultrasound, intranodal lymph angiography, dynamic contrast magnetic resonance lymphangiography etc.
