Why Gonadotropin stimulation (GnRH) Test?

CLINICAL INFORMATION

Gonadotropins also called chorionic gonadotropin or LH RH: Luteinizing Hormone-Releasing Hormone (Gonadotropins = LH-Luteinizing Hormone and FSH – Follicle Stimulating Hormone) are hormones secreted by the cells of the anterior pituitary. LH and FSH are called gonadotropins since it triggers the gonads i.e testes in males and ovaries in females. Neurons of the hypothalamus secrete the GnRH (a peptide hormone) that stimulates the downstream production of sex hormones by the gonads and thus regulates its function such as puberty onset, sexual development, ovulatory cycles in females and spermatogenesis in males etc. Gonadotropin-Releasing Hormone (GnRH) is the key regulator of the reproductive axis. Thus the pulsatile secretion of GnRH determines the function of its other related hormones like Testosterone etc. Hence this cascade triggers and regulates its target cells (i.e in males, the LH binds to the receptor cells in the testes) and results in gamete maturation in gonads for the development of sperms in males and in females it stimulates the ovaries to produce follicles (i.e oocyte or egg). Gonadotropin-Releasing Hormone (GnRH) Stimulation Test is performed to detect the function of the pituitary gland. This test helps to identify the early or delayed cause of puberty (Leuprolide Stimulation Test) for the diagnosis of precocious puberty i.e onset of secondary sexual characteristics etc, fertility treatments like intrauterine insemination or in-vitro fertilization, initiate early menstrual cycle to cause multiple eggs and to grow and mature in its size, cause of hypogonadism (to differentiate its cause - if it is due to GnRH or its origin is due to high-stress levels along with elevated dopamine and prolactin and also helps to detect prostate cancers in males etc, so as to aid in the definitive diagnosis, to help in treatment modalities. In Men, lack of gonadotropins affects the growth of testes and thus have relatively less sperm due to lack of testosterone and while in females, decreased secretion of gonadotropins in the production of immature eggs (oocytes) as they lack female hormones such as estrogen and progesterone.  Other additional tests include estimation of estrogen, testosterone, progesterone etc, ELISA Test for autoantibodies (autoimmune antibodies) and imaging studies such as CT Scan, MRI etc.
