Why Anti-Scl-70 Antibody (Anti-Topoisomerase-1) test?
Scl 70 also known as topoisomerase 1 is a nuclear and nucleolar enzyme. The presence of anti-topoisomerase antibodies inhibits the enzyme activity of Scl 70 (topoisomerase 1). Anti-Scl-70 Antibody (Anti-Topoisomerase-1)is also called an anti-DNA topoisomerase-1 antibody (anti-topo-1). Antibodies against Scl 70 are considered to be specific for scleroderma - connective tissue disease caused due to overproduction of collagen in affected areas hence also has proportionally increased density of topo-isomerase sites in collagen genes and therefore the antibodies may alter the transcription at these gene loci (HLA-DR2 –DR 15 and DR16 are associated with scleroderma and systemic sclerosis), extensive cutaneous involvement and interstitial pulmonary fibrosis. Autoantibodies against topoisomerase-1 (anti-Scl 70 antibodies) are specific serological markers of diffuse cutaneous systemic sclerosis (dcSSc), scleroderma (CREST syndrome more associated with anti-centromere antibodies, and/or systemic sclerosis characterized by microvascular damage and fibrosis of the skin and visceral organs such as kidneys causing nephritis). Evaluating patients with signs and symptoms of scleroderma and related connective tissue disease are usually tested positive for antinuclear antibodies.
