Why Alkaline phosphatase (ALP) – Bone Fraction test?

CLINICAL INFORMATION :

Alkaline Phosphatase (ALP) in Bone Fraction is usually performed in bone diseases and other related conditions like leukemia, hyperparathyroidism, etc. ALP bone iso-enzyme test is a blood test that measures the level of ALP-2 in the bones. This test indicates screening of abnormal bone growth, its associated bone diseases, and other related bone pathologies related to hormones and mineral metabolism like Paget’s disease, kidney disease, etc. Hence serum ALP-2 is a tumor marker with high specificity in osteosarcoma. Bone ALP-2 is a major regulator of bone mineralization, moreover, it provides inorganic phosphate for the synthesis of hydroxyapatite. Major symptoms that lead to the ALP-2 test are fatigue, weakness, poor appetite, nausea and vomiting, stomach and abdominal pain, dark urine, bone and joint pain, bone deformities, etc. Complications include gastro-intestinal inflammations (ulcerations, inflammatory bowel disease), cancer (bronchogenic, breast, colon, ovarian, cervical, prostate, etc), hyperphosphatemia (it is also associated with mental retardation hence called hyperphosphatasia with mental retardation syndrome - HPMRS), pernicious anemia, myocardial, renal and sometimes pulmonary infarction, renal failure (renal dystrophy, decreased clearance), etc. Meanwhile, as a part of routine diagnosis ALP (other isoenzymes) along with GGT helps to detect liver diseases or disorders, gall bladder, bile duct diseases, etc, as these enzymes levels are elevated in the bloodstream. ALP isoenzyme levels can also get elevated includes medications, auto-immune diseases, infiltrative liver diseases like sarcoidosis or amyloidosis, hepatocellular carcinoma, bile duct strictures, etc.
